Booking, Coverage & Rides

Call 920-397-7624 Monday through Friday, 8:00 am to 5:00 pm, or use our Request a Ride form. Please book at least 24 hours ahead. Standing rides for dialysis or therapy can be set up once for the whole schedule.

We drive Monday through Friday, 6:00 am to 6:00 pm. The office is open 8:00 am to 5:00 pm. Weekend rides are available by request; call ahead and we will do our best to accommodate you.

If you are a member of My Choice Wisconsin, IRIS, Community Care or Inclusa, transportation is often covered through your care plan. Ask your care manager to authorize LifeCare Transport. Not covered? We offer flat-rate private pay.

Our vans fit manual and power wheelchairs up to 34 inches wide. If your chair is larger or you use a stretcher, call us and we will talk through options.

Yes. One companion or caregiver can ride along at no charge. Just let us know when you book so we plan seating.

Yes, for shorter appointments. For longer visits such as dialysis we schedule a return pickup so you are never stranded.

We cover Fort Atkinson, Jefferson, Watertown, Janesville, Waukesha, Brookfield, Milwaukee, Menomonee Falls and surrounding communities. See our Service Areas page or call to confirm.

Call as soon as you know. We ask for at least 2 hours’ notice so we can reassign the driver.
