Rides Through Your Care Plan
If you are enrolled in Family Care or IRIS, transportation to medical appointments and community activities is often part of your plan. LifeCare Transport is an approved provider for the programs below, and we bill your managed care organization directly, so there is no paperwork on your end.

Let your care manager or IRIS consultant know you would like to use LifeCare Transport for your rides.

Your plan sends the ride authorization directly to our office. We confirm it and set up your profile, including any mobility needs.

Call 920-397-7624 with the date, time and destination. We bill your plan directly, so there is nothing to fill out and nothing to pay at the door.

Coverage varies by plan, but typically includes medical appointments, therapy, dialysis, day programs and approved community activities. Your care manager can confirm what your plan allows.
